Fill out the Client Information Questionnaire.
Once the Client Information Questionnaire has been completed:
A: Hit the "Submit Form Electronically" button at the end of the Questionnaire and the Form will automatically load as an e-mail and attachment in your E-mail service program .
B: Save the Client Information Questionnaire and send as an attachment to ggplaw@ggplaw.com
